    What are the biggest reasons forums fail and what can be done to avoid these things?

    The reason many forums fail is lack of active management or staff. If the forum admin or owner is not there daily, then forum members won't be either. You need to be on site to show your presence and then forum members will be tehre too.

    If you don't have new content on your forum daily you are likely to fail. Your members will want new discussions. Or, else they will leave to find another forum.

    Plain indolence can cause your form to fail. That means you are not doing anything and for sure your forum will suffer because a forum is a business that needs to be operated and the admin or mod have their own jobs. But as a forum owner, you still have to check on your forum to see that it is running okay particularly with the staff if they are doing their work correctly. So if you want your forum to fail then just let it be, sit back and relax with a stress free life.
