Use goes to a webpage. they see a basic page. logo, text info, then a form. they need to enter the email then name. then press buy.
They then pay using Pay-pal.
You will then use Pay-pal IPN or whatever to send user to a PHP page (using POST/GET). You send the user email and name (they entered on the form) to the PHP page. I will give you the php page URL.
I will give background for page and logo and text info.
this should be easy if you experience. I can do myself but dont have time. So maybe i can give you advice on making it.
