I need to create on online questionnaire, same as:

Questions and format 100% the same.
The questionnaire should be in PHP with mysql database.
When a visitor submit the form, his options should be registered in a database, and a notification send to the website admin.

On the other side the website visitor who filled the form should get a customized report based on his answers.
So if he selected on question 5, option 1 he will get a specified answer, but if he selected option 2 he will get another answer.
List of questions' options and answers will be provided.
