You do not use a thorough evaluation process when hiring new staff
The staff are employed by another company and because of this are not culturally connected and committed to your project
Your team are working on multiple projects at the same time and so are not focussed on your project
Your budget is too low
Your development team simply do not have the skills and experience that you need

I think that's where breaking the big project into small modules can be explained. Here you have to hire the manager. And need to work with multiple people. And then connect the dots. So you basically need to hire graphic designer from different company. And the coder from different company. And final website or project needs to be connected in small pieces. So that seems to be the way for the outsourcing works out. I have seen people working in this manner and getting things done. So always break the project into small milestones and get your team to reach it.
